Registration hours will be 9:00a.m. to 11:30 am and 1:00 p.m. to
3:00 p.m. Children entering kindergarten must be five years of
age before September 1, 2007.
Please bring the following with you at the time of registration.
A birth certificate or hospital birth registration certificate must be presented at registration.
You will need your child's social security number.
Written physicians verification showing dates of all immunizations is required at the time of registration.
The parent or guardian must provide proof of residency in the Neshaminy School
District (tax receipts, agreements of sale, or other legal documents indicating permanent address may be used as proof of
residency
IMMUNIZATION
Registration will not be considered complete until written
verification of the dates of immunization is shown. In
accordance with Pennsylvania law, children will be barred from
entering school in September, unless all of the immunizations
listed below are complete.
- Four doses of Tetanus and Diphtheria Vaccine with at least one of
those doses on or after the child's 4th birthday
- Three doses of Polio Vaccine
- Two doses of Measles vaccine
- One dose of Rubella Vaccine
- One dose Mumps Vaccine
- One dose of Varicella (or verification of chickenpox disease)
- Three doses of Hepatitis B vaccine.
