If your child requires medication to be given in school, please remember to ask you physician for a note. A note is required for any medication to be given in school. All medication given at school must be in the original container. If it is a a prescription medication, ask the pharmacist for an identical bottle for school. He will even measure out liquid medication for the amount of school days required. As a reminder, students are not permitted to transport medication to and from school. 1.
A physician’s note which clearly states: 2. Parent note giving permission to give above to their child. 3. Medication waiver signed. |
