Welcome back to a new and
exciting school year.
The School Health Law requires medical examinations for children on
original entry into school and in Grades 6 and 11, and dental
examinations for those on original entry into school and in Grades 3 and
7.
We are recommending that these examinations be done by your family
physician and dentist since they can best evaluate your child’s health
and assist you in obtaining necessary treatments and corrections.
The appropriate forms have been sent
home with your child in May, 2005. Please return these forms on or
before October 1st or call the nurse’s office with the
scheduled physical or dental date. If the forms are not received, then
your child will be scheduled to see the school physician or dentist.
The Health Office is located next to the library. Please keep us
informed of any significant health concerns (allergies, chronic
conditions such as asthma, diabetes, attention deficit disorder, etc.)
or changes in your child’s health. It is extremely important that you
keep the emergency card up to date throughout the school year.
 Wanted:
The nurse’s office is in need of any size sweatpants especially medium
sizes. If you have any please drop them off at any time. Thank you
